We ask that you arrive about 10 minutes early to your first appointment to complete our new patient forms.
Health History Form
*IMPORTANT: Any form submitted by email must be encrypted, to ensure it is secure. By submitting an unencrypted email form, you are consenting to an unsecured email. Alternatively, please print off the form, complete it, and bring it to our office.
Your First Visit
Your first appointment will be approximately one hour.
Examination
At the beginning of your appointment, your dentist will review your medical and dental history and complete a comprehensive dental exam. Your exam will include an oral cancer screening, dental x-rays, and a thorough examination of your gums, teeth, bite, and jaw.
Treatment Planning
Based on your history and examination, your dentist will develop a long-term dental treatment plan for you. Your dentist will also take the time to answer any questions you have about your dental health and treatment options.
Parking
There is parking located beneath both Canada Place and the Citadel Theatre. There are also parking meters around both buildings and pay lots located near the office.

Appointments
In order to provide a quality dental practice, appointments are required for all visits. Walk-in appointments are not accepted.
Insurance & Direct Billing
At Canada Place Dental, we accept most insurance plans. As a courtesy to you, our office will submit your claims to your insurance carrier. We offer electronic submission (EDI) where possible to expedite the claim process. When more extensive treatment is planned, we will send a detailed estimate or predetermination to the insurance carrier(s) involved.
Payment Methods
For your convenience, we accept cash, personal cheques, money orders and major credit cards including, VISA, MasterCard and AMERICAN EXPRESS. Payment is expected at the time services are performed. When more extensive dental care is necessary, financial arrangements can be made with our office.
